29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): Location: Jobsite, 202 Rosebrook Road, New Canaan, CT The employees, installing plywood to an outer wall while standing on a 4 in 12 pitch roof, were not protected from 12 foot fall hazards by the use of guardrails, safety nets, or personal fall arrest systems. Z. Stupka Carpentry, LLC was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 856943, citation number 01, item number 002a, issued on 02/26/2013 with respect to a workplace located at 9 Turkey Hill Road, Westport, and became a final order on 03/15/2013.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Location: Jobsite, 202 Rosebrook Road, New Canaan, CT Employees exposed to fall hazards of up to 12 feet above the ground while working on a 4 in 12 pitched roof were not trained in fall hazards and the procedures to be followed to minimize such hazards. Z. Stupka Carpentry, LLC was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.503(a)(1) which was contained in OSHA inspection number 856943, citation number 01, item number 002b, issued on 02/26/2013 with respect to a workplace located at 9 Turkey Hill Road, Westport, and became a final order on 03/15/2013.
