OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees were exposed to struck by and/or crushed-by hazards:     a)  Warehouse - On or about March 31, 2014, employees were exposed to struck-by and/or crushed-by hazards from pallet racks that were not anchored to the floor.    b)  Warehouse, 1st aisle - On or about March 31, 2014, the horizontal beam of the storage rack is bent and has pallets of product weighing up to 460lbs. stored on top shelf.    c)  Warehouse, East Wall - On or about March 31, 2014, the Vertical beam of the storage rack is bent and has pallets of product weghing up to 460 lbs., stored on top shelf.    One feasible method to correct the hazard includes, but is not limited to: Follow MH16.1- 2008, ANSI-RMI Specification for the Design, Testing and Utilization of Industrial Steel Storage Racks, Section 1.4.7, Column Base Plates and Anchors, which states that all rack columns shall be anchored to the floor with anchor bolts capable of resisting the forces caused by the horizontal and vertical loads on the rack; and 1.4.9 Rack Damage, which states upon any visible damage, the pertinent portions of the rack shall be unloaded immediately by the user until the damaged portion is repaired or replaced.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:   a)  Rear of warehouse - On or about March 31, 2014, a marked portable fire extinguisher was blocked by pallets of product and not easily accessible.  b)  Front of Warehouse - On or about March 31, 2014, a markerd portable fire extinguisher was blocked by cardboard boxes and not easily accessible.
