29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard. The employer does not ensure that open-sided floors and/or platforms four feet or more above adjacent floor or ground level were guarded with standard railings (or equivalent) and toeboards. This violation was observed on or about March 26, 2014 in the fabrication area of the facility where employees were exposed to a fall hazard of four feet or more when working adjacent to an open sided floor with no standard railings installed. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation describing the actions it is taking to ensure that employees are not exposed to fall hazards when working next to open sided floors or platforms four feet or more above adjacent floors.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer does not guard all the rotating parts of machines whose operation exposes employees to injury. a. This violation occurred on or about March 26, 2014, in the fabrication area where employees were exposed to struck-by and caught-by hazards when operating a YOUJI 3000ATC machine with the outer doors removed. b. This violation occurred on or about March 26, 2014, in the fabrication area where employees were exposed to struck-by and caught-by hazards when operating a YOUJI 1200ATC machine without interlocking doors. c. This violation occurred on or about March 26, 2014, in the fabrication area where employees were exposed to struck-by and caught-by hazards when operating a YOUJI 2000ATC machine without interlocking doors. d. This violation occurred on or about March 26, 2014, in the fabrication area where employees were exposed to struck-by and caught-by hazards when operating a JET GH-1660ZX Machine without a chuck guard. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the rotating parts of machinery are guarded.
