29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system, or by the use of a safety monitoring system alone: (a) Buildings by Design at 4615 State Highway 6, DeBeque, CO 81630: On or before January 17, 2014, the exposing employer did not ensure their employee was protected from injury, in that an employee engaged in framing activities on a low slope roof of a commercial building was exposed to falls of 24 feet without the use of an adequate fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Buildings by Design at 4615 State Highway 6, DeBeque, CO 81630: On or before January 17, 2014, the exposing employer did not ensure that the ladder employees were using to access an upper landing surface extended 3 feet above the upper landing area. This condition exposed the employees to a fall hazard of approximately 24 feet.
