29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) The employer did not ensure that fall protection was being used when conducting roofing work on a residential building. The employees were exposed to a fall hazard.    Water Tight Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 905963, citation number 02, item number 01 and was affirmed as a final order on June 25, 2013, with respect to a workplace location at N36 W23837 Broken Hill Circle South, Pewaukee, WI 53072.  Water Tight Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315118976, citation number 01, item number 01 and was affirmed as a final order on March 16, 2011, with respect to a workplace location at W329 S8755 S. Oak Tree Dr., Mukwonago, WI 53149.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  (a) The employer did not ensure that a ladder used for access and egress was secured to prevent displacement. An employee was exposed to a fall hazard.
