An employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) No. 7 Townhouse - On or about March 27, 2014 employees were on the edge of a flat deck framing a wall without guardrail system or safety net system or personal fall arrest system. The employees were exposed to a fall height of 18 feet.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. (a) Townhouse No. 11 - On or about March 27, 2014 employees were installing shingles on a 7/12 pitch roof at a working height of 37 feet without fall protection.
