29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    a) Coolidge Avenue and South Street, South Plainfield, NJ 07080. Employees were using an extension cord to operate their power tools when the extension cord was missing the grounding pin exposing the employees to shock hazards.   Condition occurred on or about 04/01/14.

29 CFR 1926.501(b)(1): Employee on a walking/working surface having an unprotected side or edge which is six feet or more above a lower level were not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system:  a) Coolidge Avenue and South Street, South Plainfield, NJ 07080. Employee was working on and from a balcony in excess of 30 feet above the ground with no fall protection system in place.   Condition occurred on or about 04/01/14.

Employee was operating a Rough Terrain Forklift while not wearing a seat belt exposing him to crushing hazards in the event of tip over.   Condition occurred on or about 04/01/14.

Two employees were working below active overhead construction work while not wearing any head protection exposing them to struck by hazards.     Condition occurred on or about 04/01/14.     WRTS Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 312977713, citation number 1, item number 1 and was affirmed as a final order on 05/31/2010, with respect to a workplace located at The Villas at Highgate, Lot 209.

WRTS Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 313979049, citation number 1, item number 1 and was affirmed as a final order on 05/11/2010, with respect to a workplace located at 4 Grace Court, Plainsboro, NJ 08536.
