29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) 113-128 Kelly Drive, Edison, NJ 08820. CSHO observed one employee operating a pneumatic nail gun while installing new roof skylights while not wearing any eye and or face protection exposing him to eye and face injuries from flying debris.    Condition occurred on or about 04/01/14.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) 113-128 Kelly Drive, Edison, NJ 08820. CSHO observed two employees installing new roof skylights with no fall protection systems in place exposing them to fall hazards.    Condition occurred on or about 04/01/14.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) 113-128 Kelly Drive, Edison, NJ 08820. CSHO observed an employee using a portable ladder to access the roof when the ladder did not extend three feet above the landing exposing him to fall hazards.  Condition occurred on or about 04/01/14.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with Do Not Use or similar language, and were not withdrawn from service until repaired:  a) 113-128 Kelly Drive, Edison, NJ 08820. CSHO observed an employee using a portable ladder to access the roof when the ladder was damaged in that it had broken rungs exposing the employee to fall hazards.    Condition occurred on or about 04/01/14.
