29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.   (a) Location: 110 River Road, Andover, MA 01810 On or about 3/28/14, the employer did not certify the employees who were exposed to fall hazards were provided a training program.    Glidewell Therrien was previously cited for violation of this Occupational Safety and Health standard or its equivalent standard 1926.503(b)(1), which was contained in OSHA inspection number 933764, citation number 1, item number 002B issued on 10/25/2013 and was affirmed final order on 11/12/13 with respect to a workplace located at 184 Dudley Street, Roxbury Crossing, Ma 02120.
