29 CFR 1910.23(a)(8)(ii):  Every floor hole into which persons can accidentally walk were not guarded by either standard railing with standard toe board on all exposed sides, or by a floor hole cover of standard strength and construction:      a)    On or about April 1, 2014, near the west side of the facility, the expanded metal flooring over the old waste treatment pit/area had sunk down due to the surrounding concrete being worn away resulting in exposed floor holes approximately 4 inches deep and 1 to 5 feet wide that could lead to tripping or falling hazard(s).      b)   On or about April 1, 2014, the elevated floors used to access the tanks in the three tank areas were not of a standard strength or construction in that the wood plank flooring was not secured and made solid.  The planks would move when walked upon, often separating from each other, resulting in floor openings or protruding edges that would cause fall or slip/trip hazard(s).

29 CFR 1910.1200(h)(3)(iv):  Employee training did not include at least an explanation of the labels received on shipped containers and the workplace labeling system used by their employer and the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information:  On or about April 1, 2014,  the employer had not provided the detailed training as required for such hazardous materials/chemicals as, but not limited to the nitric acid and acetone in use on site.
