At the site - employees were exposed to a 19' and 10'-4" fall hazard, in that, the use of fall protection was not enforced while working from a 5 in 12 pitched roof. Employees were removing roofing tar paper. Violation observed on or about 4/2/14.

At the site - Employees were exposed to a 10'-4" fall hazard, in that, the ladder side rails did not extend 36" above the roof landing. The ladder was extended approximately 18" above the roof landing. Violation observed on or about 4/2/14.
