At the site - employees were exposed to a 14 ft and a 10 ft fall hazard, in that, the use of fall protection was not enforced while working from a 5 in 12 pitched roof. Employees were removing and installing roofing tar paper. Violation observed on or about 4/2/14.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a. At the site - employees were exposed to a fall hazard, in that, the ladder side rails did not extend 36" above the roof landing. The ladder extended approximately 30" above the roof landing. Violation observed on or about 4/2/14.
