29 CFR 1910.141(a)(4)(ii): Wastes were not removed as often as necessary to maintain the place of employment in a sanitary condition:       (a)  Brookside Enterprises Inc., Van Wert, Ohio:  On or about April 2, 2014 at worksite located at 601 N. Cable Road, Lima, Ohio, the employer did not ensure that all waste was removed from the bathroom.  The out-of-service toilet was not cleaned and disinfected and presented a health risk to employees.
