29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a.  On or about April 2, 2014, during masonry activities the employer did not ensure all the working levels of a Waco brand fabricated frame type scaffold, located on the south eastern corner of work site was fully planked or decked between the front uprights and the guardrail supports.        b. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:                                                                                                                                                                      a.  On or about April 2, 2014, during masonry activities the employer did not ensure an employee working from a Waco brand fabricated frame type scaffold, located on the south eastern corner of the worksite,  was provided with portable ladder, hook-on type ladder, or other safe means of access to a scaffold platform.  The platform was located approximately 13.6 feet above the ground.  The employee accessed the scaffold by climbing the end frame that was not designed for ladder use.      b.  On or about April 2, 2014, during masonry activities the employer did not ensure employees working from a Waco brand fabricated frame type scaffold, located on the south side of the worksite,  were provided with portable ladder, hook-on type ladder, or other safe means of access to scaffold platforms. The platforms were located approximately 13 feet and 17 feet above the ground.  The employees accessed the scaffold by climbing the end frame that was not designed for ladder use.

On or about April 2, 2014, during masonry activities the employer did not ensure an employee working from a Waco brand fabricated frame type scaffold, located on the south eastern corner of the worksite, was protected from falling by a guardrail, personal fall arrest, or other fall protection system.  The employee was working approximately 13.6 feet above the ground.   b.  On or about April 2, 2014, during masonry activities the employer did not ensure employees working from a Waco brand fabricated frame type scaffold, located on the south side of the worksite, were protected from falling by a guardrail, personal fall arrest, or other fall protection system.  The employees were working approximately 13 and 17 feet above the ground.
