29 CFR 1910.179(g)(1)(v): Pendant control box(es) were not constructed to prevent electrical shock and were not clearly marked for identification of functions:   A.  On or about April 3, 2014, Centrifuge area:  The pendant control for the CM Shopstar hoist had a crack in the housing, exposing employees to electric shock hazards.  The control was wrapped with black electrical tape which obscured the identification of the control buttons.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  A.  On or about April 3, 2014:  1.  Passivation Area Drying Table:  A Belkin 6-receptacle relocatable power tap was energized through an extension cord, against listing and labeling instructions.  The receptacles provided power to the Dayton blower fan and Haulmaster electric hoist.  2.  At Donaldson Torit VS-1500 Dust Collector:  A Ridgid 16-Gallon Professional Wet/Dry Vac, WD19560, S/N 12335R0297 was used on a monthly basis to clean accumulated combustible cob dust from the collection drawer of the dust collector unit, against the manufacturer's instructions.
