OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment in a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards when working on elevated work platforms:   a.   Moline Forge, Inc., did not ensure that employees working from a shop made work platform were protected from fall and crushing hazards while engaged in servicing and maintenance activities.  The work platform was designed and constructed so the working level was located 78 inches above the forks, exposing employees to a tip-over hazard when the platform was raised by a powered industrial truck.  Additionally, the work platform was not marked with the weight of the empty platform, the maximum load including personnel and equipment, and the minimum capacity of the truck on which the platform can be used, which could allow a lift truck become overloaded and unstable.  b. Additionally, the work platform was not marked with the weight of the empty platform, the maximum load including personnel and equipment, and the minimum capacity of the truck on which the platform can be used, which could allow a lift truck become overloaded and unstable.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  Employees were exposed to a fire hazards because oxygen cylinders were stored next to a reserve acetylene fuel-gas cylinders within the same cylinder storage rack, without a noncombustible barrier.
