29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:   (a) U.S. Marble Construction; in the shop area, at the entrance of the the restroom. The outlet box was not covered with faceplate. Employees use the power outlet to power tools and equipment. Employees are exposed to shock hazards. The condition was noted on or about 03/27/2014.    NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1910.305(c)(4): Faceplates for flush-mounted snap switches. Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface.   (a) U.S. Marble Construction; in the shop area, at the entrance of the office. The snap switch was not covered with faceplate. Employees use the switch to turn on and off the lights in the shop and restroom. Employees are exposed to shock hazards. The condition was noted on or about 03/27/2014. 29 CFR 1910.106(e)(2)(iv)(a): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), were not kept in covered containers when not actually in use:        (a) U.S. Marble Construction, in the shop area,  flammable liquids, such as but not limited to, acetone and polyester adhesive, were used and stored. The containers were not covered. The condition was noted on or about 03/27/2014.
