29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers shall be equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails:    In Bay 1, the stair way that leads to the mezzanine level did not have a mid rail and the post for the top rail was broken off at the floor. This compromised the integrity and the strength of the hand rail. Employees were exposed to a fall hazard.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge.     (a) In Bay #8, emergency exit doors #1 and #2 were blocked by machinery and locked, preventing employees from exiting the building in case of an emergency.      (b) In Bay #7, emergency exit door #3 was blocked by propane tank and locked, preventing employees from exiting the building in case of an emergency.     (c) In Bay #5 and #6 emergency exit door was blocked by machinery and locked, preventing employees from exiting the building in case of an emergency.     (d) In Bay #2 emergency exit door #8 was welded shut and emergency exit #9 was locked, preventing employees from exiting the building in case of an emergency. 29 CFR 1910.213(g)(1): Each swing cutoff saw shall be provided with a hood that will completely enclose the upper half of the saw, the arbor end, and the point of operation at all positions of the saw. The hood shall be constructed in such a manner and of such material that it will protect the operator from flying splinters and broken saw teeth. Its hood shall be so designed that it will automatically cover the lower portion of the blade, so that when the saw is returned to the back of the table the hood will rise on top of the fence, and when the saw is moved forward the hood will drop on top of and remain in contact with the table or material being cut.    The employer failed to provide methods of machine guarding that would protect employees while operating the Rigid chop saw. The saw did not have the blade guard installed. This exposed employees to lacerations and/or amputations upon accidental contact with the rotating blade.    No abatement documentation is required for this item.
