29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) CNC Machine area - On April 2, 2014 and times prior to employees were exposed to the rotating chuck on a CNC machine where the door was left open and the operator reached into the rotating nylon part to remove the plastic shavings.
