29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   (a)  On or about March 28, 2014, on the east side of the building, employees were working on a steep roof without using any type of fall protection, exposing employees to falls of more than 6 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface being accessed:  (a) On or about March 28, 2014, on the east side of the building, employees used a Werner aluminum extension ladder with side rails that did not extend at least 3 feet above the edge of the roof to access the roof.  The employees were exposed to falls of approximately 12 feet to the ground below.
