29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about April 2, 2014, on the west side of the building, employees were not wearing hard hats while working below other employees that were engaged in cornice activities. The employees were exposed to struck-by hazards from falling objects.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section. On or about April 2, 2014, on the west side of the residential building, employees working from a pole scaffold worked without fall protection were exposed to a fall hazard of 14 feet to the lower level.

29 CFR 1926.452(a)(2): Cross bracing was not installed between the inner and outer sets of poles on double pole scaffolds. On or about April 2, 2014, on the west side of the residential building, employees working from a pole scaffold that did not have cross braces were exposed to the hazard of falling more than 14 feet to the lower level.
