29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a)  On or about April 2, 2014, at the above addressed job site, employees who had been erecting and maintaining suspended swing stage scaffolds, were exposed to a fall hazard while working on fabricated frame scaffolds that were not fully planked.      7 instances: 7 ea. fabricated frame scaffolds were erected on the roof area to support the installation and maintenance of 7 ea. suspended swing stage scaffolds, and each lacked planking on the upper level where employees had been working. 29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    a. On or about April 2, 2014, at the above addressed job site, employees who had been performing concrete restoration work from deficient suspended swing stage scaffolds and fabricated frame scaffolds, were exposed to fall hazards as the scaffolds had not been inspected by a competent person before each work shift and had the following deficiencies:    14 Instances: There were 7 ea. swing stage scaffolds that each had a fabricated frame scaffold.    1.The 7 fabricated frame scaffolds were lacking planking, and guardrails.  2. Swing stage scaffolds which had been suspended and secured by wire ropes which had clips that had not been retightened to the manufacturer's recommendations at the start of each work shift.  3. Vertical lifelines that had been used on swing stage scaffolds had been tied to anchorages with knots.   4. Single tiebacks had been installed at angles to the face of the building.      PLEASE NOTE THAT DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED.

On or about April 2, 2014, at the above addressed job site, employees who had been erecting and maintaining suspended swing stage scaffolds, were exposed to a fall hazard while working on fabricated frame scaffolds without means of conventional fall protection as the scaffolds lacked guardrails.    7  Instances: 7 three tier fabricated frame scaffolds about 5 feet wide, approximately 15 feet high that lacked guardrails on the upper level.
