29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    a. On or about April 01, 2014, at the above addressed jobsite, employees were using a tubular welded scaffolding for access/material staging. An inspection was not conducted by a competent person capable of correcting hazards such as falls, which could result in employee injuries.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:    a. On or about April 01, 2014, at the above addressed jobsite, employees using a tubular welded scaffolding for access/material staging were not trained to recognize the hazards associated with scaffolding.  The employees were thereby exposed to the hazards associated with falls.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) shall be restrained from tipping by guying, tying, bracing, or equivalent means:    a. On or about April 01, 2014, at the above addressed jobsite, employees were using a tubular welding scaffolding for access/material staging with a height to base width ratio of more than four to one (4:1). The scaffolding was not adequately secured with guys, ties and/or braces, thereby exposing employees to fall hazards.

On or about April 01, 2014, at the above addressed jobsite, employees were using a tubular welded scaffolding for access/material staging, approximately twenty (20) feet above the ground, which had frames that were not joined together vertically by using stacking pins. The employees were thereby exposed to the hazards associated with scaffolding collapse.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.    a.  On or about April 01, 2014, at the above addressed jobsite, employees using a tubular welded scaffolding, approximately twenty (20) feet above the ground, were not provided with an access ladder and were using the crossbraces as means of access. The employees were thereby exposed to the hazards associated with falls.
