29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: (a) Jobsite: On or about April 1, 2014, an employee was using a duplex receptical box at the end of an extension cord. This duplex electrical box was designed to be used in a fixed location and not at the end of an extension cord laying on the floor.
