29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:  a) Thunder Mountain Activity Center (TMAC), Fort Huachuca, AZ: On or about 1 April 2014 and at times prior thereto, employees did not wear chemical resistant goggles when they used GREASECUTTER PLUS and MARC 50 BOWL AND PORCELAIN CLEANER which can cause serious eye damage.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision:  a) Thunder Mountain Activity Center (TMAC), Fort Huachuca, AZ: On or about 1 April 2014 and at times prior thereto, the front cover on the timer for the Russell A/C unit was missing which allowed accidental contact to 120 volts.

29 CFR 1910.304(f)(1)(iv): Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel:  a) Back storage area (near bathroom), Thunder Mountain Activity Center (TMAC), Fort Huachuca, AZ: On or about 1 April 2014 and at times prior thereto, circuit breaker panel "KK" was blocked by stacked boxes on a rolling cart which prevented ready access.

Hazardous chemicals such as GREASECUTTER PLUS and MARC 50 BOWL AND PORCELAIN CLEANER are used at the activity center.
