29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Atwell & Perrine Excavating, Inc. / 186 Sylvania Road, Slippery Rock, PA: On or about April 2, 2014, an employee was in an excavation performing sewer line work for a residential property. The employee did not receive workplace specific trenching and excavation training and on the requirements noted in the 29 CFR 1926 Subpart P. Employee did not receive instruction on excavations and trenching safety items that included but were not limited to, soil inspection and frequency, warning signs of impending collapse, ladder placement and distance, spoil pile location, and when a warning system is to be used for mobile equipment.
