29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) For the Lawton Printers facility at 649 Triumph Court, Orlando, FL 32805, as observed on or about 04/03/2014. In that maintenance activities such as changing the blades for rollers on the Lithrone SX 29 Komori press were performed by employees without de-energizing the machine at a circuit breaking device such as a disconnect. An emergency stop switch (which is not an energy isolating device) was pressed instead.

The dead front pressure was measured to be 60 p.s.i.

No certification record of forklift operators training was available. Employees operated a sit down Hyster electric forklift and a stand up electric pallet jack. No records of forklift training were available to show that employees had received training which met the requirements of paragraph (l).

The following electric equipment were observed to be blocked: a) An electrical switch box was blocked by a table on the rear side of the Komori Lithrone SX 29. b) Two cabinet breaker boxes were blocked by various materials next to an older Heildeberg press.
