29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: Worksite located at 6101 New Sapulpa Rd, Tulsa, OK 74131. On or about April 1, 2014, employees were observed performing leading edge work without the use of guardrails, safety nets, or personal fall arrest systems. The employees were exposed to a fall approximately 40 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Worksite located at 6101 New Sapulpa Rd, Tulsa, OK 74131. On or about April 1, 2014, employees were observed performing leading edge roofing work and were not provided with fall protection training. The employee were exposed to a fall hazard of approximately 40 feet.
