George Urban Associates Building Complex - On or about 3/30/14, an employee was observed cutting cement blocks which created silica-containing dust.  The employee did not use water to suppress the dust nor was the employee required to use water while cutting the blocks.  The employer did not provide information to the employees on operations in their work area where hazardous chemicals were present, such as but not limited to silica. George Urban Associates Building - on or about 3/30/14, an employee was observed saw cutting cement blocks creating a large bolus of dust during the saw cutting activities.  The employer had not trained the employees regarding the methods and observations used to detect the presence or release of a hazardous chemical (silica) in the work area.
