29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):     The employer does not guard the point of operation of machines whose operation exposes employees to injury.        a. This violation was observed on or about April 4, 2014, in the production shop where employees were exposed to a crushed-by hazard while operating a Pexto shear without guarding the point of operation.      b. This violation was observed on or about April 4, 2014, in the production shop where employees were exposed to a crushed-by hazard while operating a Beader Machine without guarding the point of operation.      c. This violation was observed on or about April 4, 2014, in the production shop where employees were exposed to a crushed-by hazard while operating a Lock Former without guarding the point of operation.      d. This violation was observed on or about April 4, 2014, in the production shop where employees were exposed to a crushed-by hazard while operating a Circle Shear without guarding the point of operation.      e. This violation was observed on or about April 4, 2014, in the production shop where employees were exposed to a crushed-by hazard while operating a Duro Dyne Pinspotter without guarding the point of operation.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the point of operation of machines whose operation exposes employees to injury is guarded.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    The employer does not guard pulleys which are 7 feet or less from the floor or working platform. This violation was observed on or about April 4, 2014, on the Pexto Circle Shear where operators were exposed to a caught-in hazard when operating the machine without having a guard on the pulleys.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all pulleys 7 feet or less from the ground are guarded.

This violation was observed on or about April 4, 2014, on the Pexto Circle Shear where operators were exposed to a caught-in hazard when operating the machine without having a guard on the belts.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all belts 7 feet or less from the ground are guarded.

This violation was observed on or about April 4, 2014, in electrical panel 3 where employees were exposed to an electrical hazard when accessing the breaker panel without blanks being installed where there were unused openings.      b. This violation was observed on or about April 4, 2014, in the 3 phase panel where employees were exposed to an electrical hazard when accessing the breakers without blanks being installed where there were unused openings.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all unused openings are effectively closed.
