29 CFR 1910.177(d)(2): The employer did not provide a restraining device or a barrier to be used when inflating tires on single piece rim wheels which are not bolted onto the vehicles:    The employer does not provide tire cages to accommodate all the various size tires employees inflate.  This violation most recently occurred on or about April 3, 2014, at the establishment located at 1902 South Park Avenue in Stuttgart, Arkansas, where a tire cage was not provided to accommodate 28" tires.  While in the trajectory, an employee inflated a 28" Dee Stone tire outside a cage or barrier and the tire exploded.  This practice resulted in the employee being struck by the exploding tire which caused fatal injuries.
