29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain shall be rigged and supported in such a way that its lowest point (including sag) is no less than 34 inches (.9 m) from the walking/working surface and its highest point is no more than 39 inches (1.0 m) from the walking/working surface:  a. On or about April 2, 2014, at the above addressed jobsite, the warning line was sagging to within 10 - 18 inches in some areas, thereby exposing employees to fall hazards.

29 CFR 1926.502(h)(1)(iv): The safety monitor shall be close enough to communicate orally with the employee:  a. On or about April 2, 2014, at the above addressed jobsite, employees performing roofing work at the edge of the low sloped roof were exposed to falls of five stories due to a lack of a safety monitor to warn them of the hazards.
