29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment: On or about 4/10/2014, the employer failed to establish and follow an inspection of the safeguards of press #258. The multiplier unit of the pullbacks failed due to a worn down gear and rod inside the multiplier unit.
