29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  (a) Walmart Camp Hill Pennsylvania, Restrooms  - An employee used an extension cord to provide power to a Ryobi circular saw.  The extension cord cable was pulled from the female cord cap near the saw.  The strain was not prevented at the terminal screws inside the cord cap, on or about April 4, 2014.  No abatement certification or documentation required.
