29 CFR 1910.1025(d)(8)(ii): When the results of exposure monitoring indicated that the representative employee exposure to lead, without regard to respirators, exceeded the permissible exposure limit, the written notification did not include a statement that the permissible exposure limit was exceeded and/or a description of the corrective action taken or to be taken:   a) On or about 4/4/14, at the above address jobsite, the employer did not include a statement and description of corrective action as required in the standard when providing air sample results to employees sorting brass that exceeded the lead permissible exposure limit (PEL).  b) On or about 4/4/14, at the above address jobsite, the employer did not include a statement and description of corrective action as required in the standard when providing air sample results to employees cleaning gun ranges that exceeded the lead permissible exposure limit (PEL).

29 CFR 1910.1025(f)(2)(i): The employer must implement a respiratory protection program in accordance with 1910.134(b) through (d) (except (d)(1)(iii)), and (f) through (m), which covers each employee required by this section to use a respirator:      On or about April 4, 2014, at the addressed above jobsite, the employer permits employees to interfere with the face to facepiece seal when doning their tight fitting half face piece respirators over a rag worn on their heads, while performing brass sorting tasks.
