29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person. On or about April 04, 2014, three employees had there fall protection systems attached to a wire rope guard rail system that was not capable of supporting at least 5,000 pounds per employee, the fall protection was not installed under the supervision of a qualified person. All three employees were attached to the same wire rope guard rail.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about April 4, 2014, employees working on the building under renovation had not received training on the use of fall protection prior to being allowed to work in elevated places, exposing the employees to fall hazard of approximately 20 feet.
