One or more methods of machine guarding such as barrier guards, two-hand tripping device, electronic safety devices, were not provided to prevent employees from having any part of their body in the danger zone while a rotating conveyer on the output side of the GHL Packaging (shrink wrap) machine was in operation. This condition exposed the employees in the printing room to injuries caused by unprotected rotating parts and ingoing nip points.
