29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or are likely to be present, which necessitated the use of personal protective equipment (PPE): a) SBM Management Services, LP @ 1500 E. Route A, Wentzville, MO 63385: On and before April 4, 2013, the employer did not adequately assess a job task which involved employee exposure to a corrosive chemical, Chemico Systems 2110 Power Wax Stripper. The 2110 Power Wax Stripper had a pH of 12.5-13.5. The employers task information sheet (TIS) failed to recognize all of the routes of exposure and therefore, did not adequately prepare employees for exposure to the corrosive chemical. The products safety data sheet recognizes the primary routes of exposure as eye and skin while the employers TIS only recognizes inhalation as a route of exposure.

29 CFR 1910.147(c)(4)(ii)(D): The employer did not implement specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: a) SBM Management Services, LP @ 1500 E. Route A, Wentzville, MO 63385: On and before April 4, 2013, employees entered robot cells in the Body Shop prior to verifying the effectiveness of the installed lockout devices. Employees entered the robot cells to perform industrial cleaning to include, but not limited to, welding dust removal, sealer bucket removal/installation, and to install foil on the floor. The employer did not ensure that stored energy was released prior to employee exposure to struck-by hazards from automated robots.
