29 CFR 1910.27(b)(1)(ii): The distance between rungs exceeded 12 inches and were not uniform throughout the ladder: On and before April 7, 2014, on the outside of the loading docks in the truck terminal located at 2800 Brecksville Rd., Richfield, Ohio 44286, the employer allowed employees to use fixed ladders, to enter and exit the building, which have a ladder rung spacing of 15.6 inches.

On and before April 7, 2014, on the outside of the loading docks in the truck terminal located at 2800 Brecksville Rd., Richfield, Ohio 44286, the employer allowed employees to use fixed ladders, to enter and exit the building, which do not have a ladder rung level with the landing surface of the loading dock.
