29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   (a) Jobsite, 29 Wible Run Road, Pittsburgh, PA 15209: On or before April 7, 2014, an employee was working unprotected and exposed to fall hazards ranging from approximately 16 to 25 feet while installing top chord purlins on a 4/12 pitched roof.

29 CFR 1926.503(c)(3): When the employer has reason to believe that any affected employee who has already been trained does not  have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee:   (a) Jobsite, 29 Wible Run Road, Pittsburgh, PA 15209: On or before April 7, 2014, employees were unaware that fall protection equipment was necessary while installing purlins and exposed to fall hazards ranging between approximately 16 and 25 feet.
