29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) International Paper, K-2 Digester on or about February 16, 2014 the employer did not implement a lockout procedure for the trim circulation switching valves (pneumatic butterfly valve) before their removal, exposing employees to crush-by and caught-in hazards.
