Employees were using corrosives such as, but not limited to, Holcim Type S Masonry Cement.

29 CFR 1926.51(f)(1): The employer did not provide adequate washing facilities for employees engaged in the application of paints, coatings, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees.  Such facilities were not in near proximity to the worksite and was not equipped as to enable employees to remove such substances:  a)  At the worksite where the employer did not provide washing facilities with non-alkaline soap and hand towels for employees using Holcim Masonry Cement Type S a corrosive containing Portland Cement.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  a)  At the worksite where employees were workiing from a tubular welded scaffolding system that was not provided with ladders or other approved means of accessing platforms. Crossbraces were used as a means of access.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:   a) At the worksite where inspections of the scaffolds were not performed by a competent person, in that the designated competent person did not know that tubular welded frame scaffolding over 10 feet in height must be equipped with guard rails on the ends of the frames.
