29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): A.) Paris Companies, Inc. dba Paris Healthcare Linen Services, 100 Tom Mix Road, DuBois, Pennsylvania 16801, on or about April 7, 2014 and times prior thereto: Employees were performing servicing and maintenance on machinery including, but not limited to, a 300lb Milnor Dryer, a 12 mod tunnel, and a 160lb washer. The servicing and maintenance activities on the equipment included, but were not limited to, repairs and preventative maintenance. The lockout/tagout procedures for the machinery were missing key elements, including, but not limited to, specific language/techniques to verify deenergization, types of lockout/tagout devices necessary for energy isolation, and location / specific identification of isolation devices. B.) Paris Companies, Inc. dba Paris Healthcare Linen Services, 100 Tom Mix Road, DuBois, Pennsylvania 16801, on or about April 7, 2014 and times prior thereto: Employees were performing servicing and maintenance on cord-and-plug machinery including, but not limited to, personnel dryers. The servicing and maintenance activities on the equipment included, but were not limited to, repairs and preventative maintenance. The lockout/tagout procedures for the machinery were missing key elements, including, but not limited to, a specific procedural element / note to ensure exclusive control of the cord-and-plug device is maintained.

Paris Companies, Inc. dba Paris Healthcare Linen Services, 100 Tom Mix Road, DuBois, Pennsylvania 16801, scrub area, on or about April 7, 2014 and times prior thereto: Flexible cords were utilized to power equipment including, but not limited to, Honeywell fans (M/N: HT-900-LWS) and a tying machine. The flexible cords were running along floor surfaces and were being utilized as permanent wiring of the building. B.) Paris Companies, Inc. dba Paris Healthcare Linen Services, 100 Tom Mix Road, DuBois, Pennsylvania 16801, scrub area, on or about April 7, 2014 and times prior thereto: Flexible cords were utilized to power equipment including, but not limited to, Honeywell fans (M/N: HT-900-LWS). The flexible cords was observed running through a Braun scrub piece folder.
