29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about 04/08/2014 employees at the Mangiafico Bakery construction project were exposed to approximately 10' falls while engaged in carpentry activities.  The employees were working from the roof without fall protection.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  Employees at the Mangiafico Bakery construction project were exposed to approximately 10' falls.  Employees ladder did not extend at least three (3) feet above its landing.
