29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from fall objects, or from electrical shock and burns:      (a)UNICORP, INC. @ 970 Colorado Ave., Carbondale, CO 81623: On and before April 1st 2014, as part of the injury/accident prevention responsibilities, the exposing employer did not ensure employees conducting work in an apartment building under construction were wearing hard hats. This condition exposed employees to a possible danger of head injury from impact, or from falling or flying objects from elevation.

29 CFR 1926.403(b)(1)(i):  Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification for that identified purpose:    (a)UNICORP, INC. @ 970 Colorado Ave., Carbondale, CO 81623: On and before April 1st 2014, as part of the injury/accident prevention responsibilities, the exposing employer did not ensure that 120-volt relocatable power tap, which was being used to power a portable drill used to mix mud, was used in accordance with the instructions included listing or labeling, or certification of that identified equipment purpose and location.  The condition exposed employees to electrical shock, burns, and electrocution hazards.    Abatement Note: Relocatable power taps are intended to be directly connected to a permanently installed branch circuit receptacle. Relocatable power taps are not intended to be series connected (daisy chained) to other relocatable power taps or to extension cords. Relocatable power taps are not intended to use at construction sites and similar locations. Relocatable power taps are not intended to be permanently secure building structures, tables, work benches or similar structures, nor are they intended to be used as a substitute for fixed wiring. The cords of relocatable power taps not intended to be routed through walls, windows, ceiling, floors or similar openings Relocatable power taps are intended for indoor use to power laboratory equipment, home workshops, home movie lighting controls, musical instrumentation, and to provide outlet receptacles for computers, audio and video equipment, and other similar equipment.

29 CFR 1926.451(g)(1):  Each employee on a scaffold 10 feet above a lower level was not protected from falling to the lower level:      (a)UNICORP, INC., @ 970 Colorado Ave., Carbondale, CO 81623. On and before April 1st 2014, as part of the injury/accident prevention responsibilities, the exposing employer did not ensure that each masonry employee conducting work on the fabricated frame scaffold was protected from falling from elevation. Guardrails and end rails and/or other suitable fall protection systems were not provided on each working platform of the scaffold. This condition exposed employees to scaffold fall hazards of approximately 10 feet 8 inches to the ground.

This condition exposed the employees to the hazard of falling approximately 10 feet.
