29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket. On or about April 7, 2014 employees were exposed to fall hazards in excess of six feet when an employee climbed up on to the mid-rail of the aerial boom man basket to work on the frame of the structure without fall protection.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). On or about April 7, 2014 an employee was exposed to fall hazards in excess of six feet when an employee did not have their harness secured to the boom lift.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about April 7, 2014, employees were exposed to fall hazards in excess of six feet. Instances: 1. Employee was working in roof gables above the second level decking in excess of six feet without fall protection. 2. Employee standing on the second level decking edge in excess of six feet without fall protection
