29 CFR 1910.26(c)(1): Good safe practices in the use and care of ladder equipment were not employed by the users to get maximum serviceability, safety, and to eliminate unnecessary damage of equipment:  Hill Phoenix, Inc. - 709 Sigman Road, NE, Conyers, GA -  Rack Area - On or about April 30, 2014, an employee was allowed to work with one foot supported by a step ladder and other foot supported on the structural framing for an electrical enclosure panel that did not have a dead front cover to protect the employee form exposed energized circuit parts. Thereby exposing the employee to a fall/electrocution hazard due to the employee's unstable position on the step ladder.

29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily apparent:  Hill Phoenix, Inc. - 709 Sigman Road, NE, Conyers, GA - Brazing Department - On or about April 30, 2014, employees were exposed to burn hazards while walking and working near the exposed hot metal surface of copper piping and equipment that was left unguarded after braze welding was used to connect metal joints.  In that, a braze welding operation left copper piping and equipment in the workplace with surface temperatures that ranged between 269-420 degrees Fahrenheit.

29 CFR 1910.252(a)(2)(xiii)(C): The employer did not recognize its responsibility for the safe usage of cutting or welding equipment, and did not insist that cutters or welders and their supervisors were suitably trained in the safe operation of their equipment:  (a) Hill Phoenix, Inc.- 709 Sigman Road, NE, Conyers, GA - Welding/Brazing Area-On or about April 30, 2014 employees were exposed to a fire/burn hazard. The employer did not ensure that employees could recognize safe welding practices such as, but not limited, welding/brazing adjacent to employees with no eye protection; holding welding electrodes in a manner that others can come in contact with electrodes while not welding; and keeping wires, hoses and other equipment off the floor creating a tripping hazard.

1910.303(g)(2)(i) Live parts of electrical equipment operating at 50 volts or more was not guarded against accidental contact by the use of an approved cabinet or other form of approved enclosure, in that an employee was exposed to energized circuit elements while working near an opened  electrical panel.  Hill Phoenix, Inc. - 709 Sigman Road, NE, Conyers, GA - Rack Area - On or about April 30. 2014, an electrically unqualified employee was allowed to work near an enclosure panel that did not have a dead front cover, thereby exposing the employee to energized circuit elements. The employee was not protected by a work practice such as, the use of electrical protective equipment (e.g. barriers, rubber insulating blankets, covers, gloves and sleeves (qualified persons only), or other approved handling equipment.
