Tampa (FL) where a guard railing system designed by the employer for the Raymond Order Selector platform that had a top rail measuring 42.5" and a mid rail near the operations panel area measuring 18", as observed on or about April 8, 2014. b) for the establishment doing business as Alro Metals Service Center located at 10223 Woodberry Rd. Tampa (FL) where a guard railing system designed by the employer for the Raymond Order Selector platform section had top rails measuring 42.5" and a mid rail measuring 24.5", as observed on or about April 8, 2014. c) for the establishment doing business as Alro Metals Service Center located at 10223 Woodberry Rd. Tampa (FL) where a guard railing system designed by the employer for the Raymond Order Selector platform had top rails measuring 42.5" and a mid rail section measuring 30.75", as observed on or about April 8, 2014. d) for the establishment doing business as Alro Metals Service Center located at 10223 Woodberry Rd. 29 CFR 1910.132(f)(1)(iii): Employee(s) required to use PPE by this section were not trained to know how to don, doff, adjust, and wear PPE: a) for the employer doing business as Alro Metals Service Center located at 10223 Woodberry Rd. Tampa (FL) where an order selecting employee that was required to utilize a fall arrest harness and lanyard while operating a Raymond Order Picker had attempted to don the harness incorrectly. The employee was observed stepping into the harness backward to where the D ring was in the front of their torso, on or about April 8, 2014.
