OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards.    a. At the workplace, in the Bigfoot 9H Well area, on or about April 5, 2014, an employee was standing on the top cap of an eight foot fiberglass A-frame self-supporting ladder while unscrewing a hoisting device from the top of a blow-out preventer. The employee fell backwards off of the ladder and onto the composite floor system below.

29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.    a. At the workplace, in the Bigfoot 9H Well area, on or about April 5, 2014, a BOP device and attached equipment was being improperly stored on a rack when it fell onto an employee.  The rack was designed to only hold the BOP device and not the BOP with attached equipment assembly.
