A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet or more in depth so as to require no more than 25 feet of lateral travel for employees:    a) Sussex Correctional Institution, Worksite - employees were performing work within a trench excavation, well in excess of four feet in depth and 40 feet in length, that was not provided with an appropriate stairway, ladder, ramp or other safe means of egress, observed on or about April 1, 2014.
